  • Questions are immutable — to change wording or scoring, create a new version.
  • When replacing a question, add a lineage reason so analytics roll up across versions.
  • Publish a question set with an effective date; reviews snapshot the set active at runtime.
  • Retire a set with an end date to close it without altering historical reviews.
